Interoperability of the history of patient’s treatments records inside heterogeneous infrastructure of electronic health records

Abstract
Common "backbone" of information about the progress of the healing process (medical procedure) is a description of the clinical path for the specified disease and profiled for an individual patient. This description contains the history of the medical procedure steps already performed, as well as those which, in accordance with the best medical knowledge still need to be made. The steps are linked to specific artifacts that can occur during medical procedures performed. One type of such artifacts is electronic health records. The study presents the idea that, consistent delivery of medical records to the patient, the doctor and the other, it is possible on the basis of the history of the individual steps of clinical paths, associated with a particular patient, the disease entity, and electronic health records.
